1. A.) AFFIDAVIT OF SUPPORT Form I-864 (This must be the original document, signed, by petitioner and notarized). SEE LINK BELOW IF you do not meet the US financial requirements for supporting the adopted child, you will need to have a Joint sponser who agrees to support the child. The 2nd sponser will have to submit Form I-864 with Items 2,3,and 4 as listed below. SEE LINK BELOW THIS MUST BE THE ORIGINAL AFFADAVIT, NOTARIZED __________________________

2. AFFIDAVIT OF SUPPORT Form I-864 A- This is the form for the spouse of the petitioner if employed.

3. PROOF OF EMPLOYMENT

   This must be a letter from the employer-employers. IF YOU ARE SELF EMPLOYED you will be required to submit a copy of your business license and supportintg documents to show the ligitimacy of your business.

   THIS MUST BE A CURRENT LETTER. THIS DOES NOT HAVE TO BE NOTARIZED.

4. TAX DOCUMENTS This must be the  current year`s 1040 tax forms, including all schedules. A FAMILY WILL NEED COPIES OF THE 1040 THAT THEY FILED FOR THE CURRENT YEAR. .*PLEASE NOTE: ANY FAMILY WHO SHOWS A LOSS ON THEIR IRS TAX DOCUMENTS  WILL BE REQUIRED TO SHOW AUTHORITIVE PROOF OF NET WORTH. *IF YOU HAVE LOST YOUR TAX RETURN PAPERS YOU MAY SUBMIT AN IRS LETTER 1722 IN LEIU OF YOUR 1040 TAX RETURN. THE LETTER MAY BE A PHOTO COPY AS LONG AS THE IRS STAMP IS CLEARLY SHOWN.

5.  W2 FORMS This must be the current year1s  W2 form. THIS WOULD BE PHOTO COPIES OF ORIGINAL W2.

6.  US PASSPORTS=A PHOTO COPY OF BOTH HUSBAND AND WIFE`S PASSPORTS SHOWING PASSPORT PHOTO AND SIGNATURE.(Whether or not you are planning to travel abroad. you will need to have a current passport}. THIS WOULD BE A PHOTO COPY OF THE ORIGINAL PASSPORT *PLEASE NOTE: THIS REQUIREMENT IS FOR ALL FAMILIES, WHETHER OR NOT THEY PLAN TO TRAVEL TO LIBERIA.

7. BIRTH CERTIFICATES-A PHOTO COPY OF YOUR CERTIFIED BIRTH CERTIFICATES (BOTH HUSBAND AND WIFE).  ORIGINAL BIRTH CERTIFICATES ARE NOT REQUIRED.

8. MARRIAGE LICENSE-A COPY OF YOUR CERTIFIED MARRIAGE LICENSE THIS WOULD BE A PHOTOCOPY OF THE ORIGINAL MARRIAGE LICENSE

9. FAMILY PHOTO-A PHOTO OF YOUR FAMILY

10. HOMESTUDY-A CERTIFIED COPY OF YOUR APPROVED INTERNATIONAL HOMESTUDY. THIS NEEDS TO BE A NOTARIZED COPY OF YOUR APPROVED HOMESTUDY FOR EACH CHILD THAT YOU ARE ADOPTING.

11. VISA PHOTOS-FOUR (4) "VISA QUALITY" PHOTOS OF BOTH HUSBAND AND WIFE. (This will total eight (8) photos in all).

12. POWER OF ATTORNEY-A NOTARIZED POWER OF ATTORNEY.The power of attorney needs to naming Maria Luyken as your Power of Attorney. 

13. DIVORCE DECREE-COPY(S) OF CERTIFIED DIVORCE DECREE IF ANY. (Please include a letter explaining the circumstances of your divorce.)

14. I-600 FORM= IF A MARRIED PARENT TRAVELS WITHOUT THEIR SPOUSE TO PROCESS THE I-600 AT THE US EMBASSY IN MONROVIA, PLEASE OBTAIN AN I-600 FORM, FILL IT OUT WITH AS MUCH INFORMATION AS YOU CAN ( IT MUST "AT THE VERY  LEAST" HAVE THE CHILD`S NAME AND BIRTHDATE, THEN MAKE SURE BOTH PARENTS SIGN IT AND NOTARIZE IT. 

15. WACSN RELEASE OF RESPONSIBILITY FORM, SIGNED BY BOTH PARENTS.

16. WACSN ADOPTED CHILDREN`S UPDATE FORM, SIGNED BY BOTH PARENTS AND NOTARIZE *ALL FAMILIES TRAVELING TO LIBERIA ARE REQUIRED TO HAVE AN VALID AND CURRENT AMERICAN PASSPORT AND A LIBERIAN VISA.

17. WACSN PARENTAL ACCEPTANCE FORM-SIGNED BY BOTH PARENTS AND NOTARIZED

18. WACSN FAMILY INFORMATION SHEET-SIGNED BY BOTH PARENTS

19. A copy of your I-600A approval letter
20. A copy of your Finger Print Clearance Form Letter
21. A copy of your I-600 approval letter (if you file your I-600 in the US instead of Liberia).

Your completed dossier must be Federal Expressed to WACSN`Liberia.
