These are the required documents that Sister Maria will need to complete your Liberian Adoptions
A copy of each document must be submitted for "each" individual child that is being adopted by adopted family.( A SEPERATE DOSSIER FOR EACH CHILD YOU ARE ADOPTING WITH ITDENTICAL PAPERS AS REQUIRED ....NO EXCEPTIONS.)
- HOMESTUDY-A CERTIFIED COPY OF YOUR APPROVED INTERNATIONAL HOMESTUDY. THIS NEEDS TO BE A NOTARIZED COPY OF YOUR APPROVED HOMESTUDY FOR EACH CHILD THAT YOU ARE ADOPTING.
- POWER OF ATTORNEY-A NOTARIZED POWER OF ATTORNEY.The power of attorney needs to name both Maria Luyken & Earnest Urey as your Power of Attorney.
- WACSN RELEASE OF RESPONSIBILITY FORM, SIGNED BY BOTH PARENTS.
